Physical Therapy Evaluation and Treatment (first visit) - $120: The initial evaluation gives us ample time to sort out the root causes of your pain and movement dysfunction, and make long-lasting changes not possible in a traditional setting. PLEASE NOTE, I AM NO LONGER ACCEPTING INSURANCE DIRECTLY. Superbills will be issued for you to be reimbursed.
Follow-up Visit - $120: Follow-up visits are a great way to progress your pain-free movements, as all of the perpetuating factors were identified during the initial evaluation. PLEASE NOTE THAT A $2.75 FEE WILL BE ADDED TO ALL CARD PAYMENTS.
