When scheduling, please be thorough when completing the reason for your appointment. This helps us and your provider prepare. You have the option of scheduling a virtual appointment, but your provider might convert/reschedule your appointment to an in-person based on the symptoms/circumstances.

**We reserve the right to cancel and reschedule your appointment based on your progress with your previous treatment plan. ie If your planned follow-up is two months and you are scheduling two weeks later, please let us know why. Please understand, that our appointment slots are limited to medical necessity and we want to be available to all of our members.
